| Ticket number | The board prefix and ticket number identify the complaint. |
| Status | The current label from the board's status list. |
| Due date | The target date for the ticket. Read the incident date in the form answers. |
| Assigned to / Created by | The assigned person and the ticket creator. |
| Form answers | Read the report's answers under their section headings, including location, date, and time where supplied. |
